The changes that are made to the IT environment can result in incidents which can quickly go out of control and cause a major impact on the company. Sometimes, changes do often result in unintended consequences even though Information needed to properly evaluate the potential impact of a change is unavailable or inaccurate even though companies may have good change Management by ITIL standards such as minimal unauthorized changes, regular CAB meetings, and a well-defined approval processes.
It may be partly or wholly due to the reason that they assess changes based on procedural correctness as opposed to potential risk and impact, unavailability or inaccuracy to properly evaluate the potential impact of a change, or inadequate testing pertaining to changes and releases owing to a lack of suitable non-production testing environments or a perceived lack of time.
