ITIL does not provide any standard Templates because every service is different and every organization has its own Public and Private Practices

Prepare a Template on your own however ensure to capture the below

1.Review Current Capacity

2.Improve Service and also the Component Capacity

3.Assess ,Agree,Document all new requirements

4. Finally plan capacity

Ensure that each one above has a tracker so that you can plan things much better
