Hi all

First off, I apologies if this has been asked before, I did have a look through previous topics and couldn't see anything obvious, and its been a long week, hence the title of my request!

My question is, what repercussions do you have for failed and unauthorised RFC's. Here at my firm my colleagues appear to have had a lobotomy over the last few weeks and we have had a lot of failed RFC's, for example, overrunning because they haven't thought to download a software update before starting the work or leaving site after a network switch update without thoroughly testing to name a few.

I'm quite interested to hear what level people go to. I need to implement something to address these issues and have management support, just need to come up with what measures to take and thought asking here would be a useful insight.

That would depend on the organisation and the reason it failed

any where from a finger wag and saying bad bad deployment
to a deep review where the team and its members are shown their error to firing

There has to be of course - a failed change review initiated by CM

In my company every failed change record gets applied to a month scorecard, detailed out to each specific person to show performance. This clearly shows a performance difference between the dev individuals as some show sparkly clean and pretty while it shows others to be not so much...

This report is prepared by CM for upper management teams review. However this is where CM stops for our company. As much as I would like to see repercussion for having repeat issues where your performance should probably be looked at I see if being upper management's choice whether or not to discipline.

I do my part to show the level of work with pretty fine detail but its not up to me (thankfully) to go after people. However I certain voice my opinion on certain people trying to push things in. Those reports get whipped out pretty fast to show the reason why we shouldn't push forward.
