We cannot share an exact document for transtition planning as in such as each Organisation has their own limitations . create Excels/WordPads for each and every function below to begin with

Transition Plannnig Support :should contain Release policy ,Unique Identification Numbers ,Names ,Roles & Responsibilities ,Frequency ,Exit Criteria etc ..

Change Management: Request for Change ,Classify the Change ,Back Out Plans ,Service Availability ,Change Advisory Board ,who raised chnage ,reason for Change , Return Required ,Risks Involved ,Resources ,Test build etc ..

Service asset and Configuration Management , Have All Configurable Items at one Database ,Organisational CIs ,Internal CIs

Release and Deployment : Releases ,Type of Releases ,Release Methods ,Escalation Etc ,,..

Change Evaluation -- CAB Approvals ,Deviations ,Risk Profile ,Validations ,Recommnedations

Knolwedge Management -- Have all the Knowledge Aquired from the previous Experiences ,Create Known Error Data Base ,Kind of Skills for the Employees ,Access level for Employees Etc ..
