Seriously, I doubt very much if there is an abundance of problem management templates in the public domain.
Hints and tips, I can do:
1. define the purpose, objectives and scope of your problem management system.
2. establish its policy regime.
3. identify the activities, authorities and controls required to be successful.
4. design the detailed procedures required to perform 3 under the constraints of 2 and within the framework of 1 above.
5. out of this design activity should fall any forms, lists, templates, etc. of use to you.
If you still prefer to use someone else's templates, then it will help if you also have access to their 1-5 above, so that you can see just how well they correspond with your own. Otherwise you are likely to start doing things according to templates, the purpose and appropriate context of which you will be guessing, and it is unlikely to be the same as your own.
